Covid-19 Vaccine:
1.) Parent/legal guardian must provide minor's birth certificate.
2.) Parent/legal guardian must have a government-issued photo identification.
3.) Minor must be accomapnaied by a parent or legal guardian.
4.) Guardians must present legal guardianship or power-of-attorney documents.
